Jacob N. Clark, NP

Jacob “Jake” Clark is a psychiatric nurse practitioner with Intermountain Health’s Connect Care – Behavioral Health team. He cares for adults through virtual visits, offering support that meets people where they are and honors their lived experiences.

Jake has a special passion for working with young adults navigating the transition into adulthood. He also enjoys supporting people in marginalized communities, including members of the LGBTQ+ community, BIPOC individuals, people who have been previously incarcerated, sex workers, those in non‑traditional or mixed‑identity relationships, and individuals experiencing faith or life transitions — especially when those transitions involve trauma or high‑demand groups.

Jake’s approach to care is grounded in a belief that kindness is powerful. He often references a favorite quote from Mark Twain: “Kindness is a language which the deaf can hear and the blind can see.” Patients describe him as compassionate, open‑minded, and non‑judgmental. He strives to create a welcoming, affirming space for every person he meets. Jake has experience helping people work through complex family and relationship dynamics, and he advocates for mental health awareness and social justice in the community.

Originally from Orange County, California, Jake enjoys weightlifting, soccer, traveling with his spouse, attending music festivals, podcasting, reading, video games, the occasional TV binge, and spending time with his long‑haired German shepherd, Bear‑Bear. He loves helping people set realistic goals for healthier living and feels honored to support patients as they grow in their mental and emotional wellbeing.
