I get surgery on the 25th of November. At this appointment Colby White let me know that they had not received my release form from my primary care physician. I had to call my PCP about this. They let me know that they had faxed the form over 10 days ago, but they would fax it again. I think there is a breakdown somewhere in the way that things are getting processed from one hospital to another.
