About The Provider
Passionate about providing patient-centered care, Rachel DeCosta, FNP-C, is excited to join the Intermountain Health team. Rachel earned a Bachelor of Science in Nursing and a Master of Science in Nursing from Regis University in Denver. Her career has spanned primary care, pulmonary medicine, and post-Covid care. Rachel’s clinical interests include preventive care, women’s health, and pulmonary medicine. Rachel's focus is on patients ages 12 and over.
“I encourage patients to advocate for themselves and look forward to partnering with them in their care. I believe in meeting patients where they are and helping them achieve their healthiest selves.”
As a married mom of two teenage boys, Rachel enjoys Zumba, hiking, and reading in her spare time.

I understand I'm behind on alot of things like Pap, mammogram etc. but I was there for blood pressure issues, Dr. Rachel focused on that, no lectures or being demands, but she sweetly reminded me that eventually we would get to those. Very professionally. Im so happy I found her. Like I said, a diamond.

Rachel was very personable and took a lot of the pressure off when I expressed how overwhelmed I was. It’s getting harder to find a provider who will listen to you without rushing, and having had so many unsuccessful experiences in the past, I was very hesitant coming in. But Rachel had been recommended by a dear friend and I trusted that she would be open to helping me. My visit with her did not disappoint, and I appreciated that she was willing to consider my concerns without condescending or didn’t disregard or diminish my pain due to my age as many other doctors have. She was also open to integrating input from my existing providers to form a plan for medications. Even though it was just a first visit, I sincerely appreciated Rachel investing so much curiosity in my situation and reassuring me that we’d figure it out without starting from the beginning again. I have a lot of optimism that I will be well cared for and look forward to finding relief soon.
