About The Provider
Eliza Bremer is a certified Family Nurse Practitioner practicing gastroenterology at Intermountain Health's Avenues Specialty Clinic in Salt Lake City, Utah. She supports patients and their families in Salt Lake City, North Salt Lake, and the surrounding communities, focusing on preventing disease and promoting healthy lifestyles. Her clinical interests include general GI, EoE (eosinophilic esophagitis), celiac disease, and liver conditions
Eliza enjoys complex cases that challenge her critical thinking and allow her to collaborate with other providers to find the root cause of a condition—and help patients improve based on those findings. She brings a fun personality to her work and genuinely enjoys learning about her patients' lives and the unique experiences that shaped them. Through evidence-based practice, compassion, and a personal touch, Eliza strives to achieve optimal health outcomes while building lasting, meaningful relationships.
As a provider, her philosophy of care includes considering each patient's values, beliefs, and preferences when creating their healthcare plan.
Outside the clinic, the Kaysville native loves immersing herself in outdoor activities like traveling, hiking, skiing, horseback riding, running, rafting, kayaking, and fishing. She also enjoys spending time with her family, including her nephews, nieces, and two wonderful parents, and reading and exploring new adventures.

Great at easing anxieties during a primary caregiver transition. I felt comfortable and secure that my needs can be met.

Eliza was extremely knowledgable, patient, and I feel extremely grateful for her ability to connect with me as a patient and fellow human. I know that I can trust her as a provider moving forward with my care. I am 53 years old and have the experience to know that she is a gem in the healthcare world!

Eliza Bremer, NP may be a competent clinician, but I'll never know because she seems to have too many other things to do to attend to the matter at hand. ("At hand"? Finger? See what I did there?)

She was very thorough. She brought up things in my medical history that no other doctors cared to discuss, which I really appreciated. I also appreciated how she discussed vitamins, iron levels etc. rather than being too in favor of prescriptions.
